Root Cause Analysis for Practitioners (3 days) 
  • This course is not for personnel that have no knowledge or any past experience with conducting investigations, apparent causes or root causes, or for personnel that are brand new to the industry 

  • The course uses a Harvard MBA approach:

    • Case Studies of increasing difficulty are provided to enhance an understanding of key concepts and develop proficiency using the BlueDragon Framework.

    • The Case Studies for BD-301 are geared towards the Energy industry and Department of Defense operating environments/sites (e.g. Department of Energy Laboratories and project sites, commercial nuclear plants, the Nuclear Weapons Complex, the Chemical Weapons facilities, Oil & Gas plants, Chemical plants, etc.)   

  • The course provides a quick refresher on of the fundamentals of Root Cause Analysis and Apparent Cause Analysis (a more comprehensive review is provided in BD-102)

  • For our current clients, we offer an additional option using one of our skilled facilitators: 

    • Set up your BlueDragon Framework using available information

    • Data call: identify the data needed to conduct the investigation

    • Identify the overall approach needed for Phase 1 - Data Analysis, based on the available information

    • Lines of Inquiry: identify how to develop the lines of inquiry to attack the problem from multiple perspectives 

    • Affected organizations: identify the organizations that we will need to bring into the room to participate in facilitated causal analysis sessions

    • Structure the entire investigation or root cause analysis for your team 

  • ​The above can be conducted in a workshop environment with a large group of causal analysts, so they benefit from observing and participating in the process.
